In shiatsu, the practitioner (shiatsuka) connects physically with the patient in such a way as to perceive the patient’s energy/equilibrium. Acting in this way, at the time of treating the patient, the shiatsuka has already carried out an energy diagnosis. The relationship between the practitioner (torì) and patient (ukè) is therefore bidirectional. During a shiatsu session, the patient is generally stretched out on the ground on a padded mat (tatami) or sometimes sitting on a low bench or lying on a massage table, while the practitioner remains kneeling or standing. There are full treatments comprised of set massage sequences (katà) or specific treatments for various problems.
The massage is generally very pleasant and relaxing and consists of manipulations and/or pressure held for several seconds in specific areas, alternated by a gentle mobilization of the joints. Natural mobility is never forced and force is never used to resolve any existing tension knots (unlike some forms of traditional Chinese massage and Western therapeutic massage). Equilibrium is always achieved through gradual balance.

Pressure can be applied using fingertips, knuckles, palms of the hand, soles of the feet, elbows or knees, according to the position and intensity of the massage. Direct contact with the skin is generally avoided so the patient normally wears a light dress or track suit. As opposed to other forms of traditional massage, no oils or cream are used
